Telmisartan + Hydrochlorothiazide + Amlodipine

Tablet

Triple drug combination of Telmisartan, Amlodipine, and Hydrochlorothiazide is indicated for controlling high blood pressure.
High blood pressure can damage blood vessels in several organs, which could lead sometimes to heart attack, heart or kidney failure, stroke, or blindness. There are usually no symptoms of high blood pressure before damage occurs.

  • Telmisartan belongs to a group of medicines called angiotensin II receptor antagonists. Angiotensin-II is a substance produced in your body which causes your blood vessels to narrow thus increasing your blood pressure
  • Hydrochlorothiazide- belongs to a group of medicines called thiazide diuretics, which cause your urine output to increase, leading to a lowering of your blood pressure
  • Amlodipine- is a calcium channel blocker. It lowers the blood pressure by relaxing the blood vessels

Strength
  • Telmisartan 40 mg
  • Amlodipine 5 mg
  • Hydrochlorothiazide 12.5 mg
Packing Size
  • 10 Tablets

Mechanism

Telmisartan: Blocks angiotensin II receptors, preventing vasoconstriction and aldosterone release.
Hydrochlorothiazide: Increases renal excretion of sodium and water, reducing plasma volume and blood pressure.
Amlodipine: Inhibits calcium influx into vascular smooth muscle, causing vasodilation. The combination ensures effective blood pressure control and cardiovascular protection.

Pharmacokinetics

  • Absorption: Telmisartan ~50% bioavailability; Hydrochlorothiazide well absorbed orally; Amlodipine ~60–65%.
  • Distribution: All highly protein bound.
  • Metabolism: Telmisartan minimal hepatic metabolism; Amlodipine via CYP3A4; Hydrochlorothiazide minimally metabolized.
  • Elimination: Telmisartan mainly fecal; Hydrochlorothiazide renal; Amlodipine renal and fecal.
  • Half-life: Telmisartan ~24 hrs; Hydrochlorothiazide ~6–15 hrs; Amlodipine ~30–50 hrs.

Side Effects

  • Dizziness
  • Peripheral edema
  • Headache
  • Fatigue
  • Electrolyte imbalance (Hydrochlorothiazide)
  • Rare: Hypotension

Contraindication

  • Hypersensitivity to Telmisartan, Hydrochlorothiazide, or Amlodipine
  • Pregnancy (especially 2nd and 3rd trimester)
  • Severe hepatic impairment
  • Severe renal impairment
  • Cardiogenic shock (Amlodipine contraindicated)

Interaction

  • Other antihypertensives (additive effect)
  • CYP3A4 inhibitors (increase Amlodipine levels)
  • NSAIDs (may reduce antihypertensive effect)
  • Potassium-sparing diuretics (risk of hyperkalemia with Telmisartan)

Warning and Precautions

  • Use under medical supervision only
  • Monitor blood pressure and electrolytes regularly
  • Caution in renal impairment
  • Consult physician during pregnancy and lactation
